- If you need to cancel your appointment, please give us 48 hours’ notice.
- Prior to your appointment, please be sure that your Primary Care Physician is up to date with your insurance carrier.
- Please bring the following items with you to your first appointment:
- Photo ID
- Insurance card(s)
- Copayment or payment for services rendered
- Medication(s) you are currently taking in their bottle(s)
- If applicable – Medical History Form and past medical records
- Fill out the Authorization for Release of Medical Information Form
