Requesting Copies of Your Records at Southcoast Health

As a patient of Southcoast Health, we want your health care experience to go as smoothly as possible. Easily find the information you need to request medical records.

Please allow at least five business days after receipt of your fully completed form for most routine medical record requests to be fulfilled. Urgent requests should be made by phone. Fees may apply for copies of some medical records.

Provider Requests

Medical providers and medical facilities requesting medical records should call (508) 973-3700 option 2 or fax (508) 973-3690.

Patient Requests

Requests from attorneys, insurance companies and all other non-patients must be in writing and mailed to: Southcoast Health, Health Information Management, 200 Mill Road, Suite 210, Fairhaven, MA 02719. The process outlined below is for patients only.

Patients, did you know you can access your medical records online? If you are a current patient, you can view your nonsensitive test results through MyChart, Southcoast Health’s patient portal.

How Patients can Request Hospital, Urgent Care, Oncology and Physical Therapy Medical Records

Medical records are maintained by the Health Information Management Departments for all three hospitals. To obtain a copy of your medical record:

  1. Download the Authorization for Release of Information Form in English, Portuguese or Spanish.
  2. Print and complete the authorization form to release medical records.
  3. Mail, email or fax the completed form to:

Please include your name, address, home phone number, alternate phone number (such as a work or cell phone) and email address in case we need to contact you.

A photo ID is required when picking up your medical records at the hospital.

How Patients can Request Southcoast Physicians Group Medical Records

If you need your medical records from your doctor, follow the instructions below or contact the physician’s office directly:

  1. Download the Authorization for Release of Information Form-SPG in English, Portuguese or Spanish. List your doctor’s name and office address in section 3 of the form.
  2. Print and complete the authorization form to release medical records.
  3. Mail or fax the completed form to:
