Southcoast Health at Home and Southcoast Health are profoundly touched when you name us a beneficiary in lieu of memorial flowers. You may designate your gift using the following language:
Southcoast Health at Home Tree of Life
Establish a lasting tribute for your loved one with an engraved leaf on the Tree of Life.
When the total amount memorial gifts for your loved one reach $1,000 or more, an engraved leaf honoring your loved one will be displayed on our “Tree of Life” located in the lobby of the Southcoast Health at Home administrative office in Fairhaven, MA. Leaf options are as follows:
Brass ‐ $1,000 to $4,999
Silver ‐ $5,000 to $9,999
Copper ‐ $10,000+

How it Works
You can easily make your tribute or memorial gift em linha. Simply choose the amount you wish to give and select a fund. Next, check “I would like to dedicate this gift” and specify in whose honor or memory you are donating.
You can also download the Tribute Gift Donation Form to mail your donation to the Mission Advancement Department.
Southcoast Health will notify the honoree or family of your thoughtful gift.
For contributions in support of the patients who receive treatment at Southcoast Health Cancer Care, please select the “Southcoast Health Nicole Podkowa Patient & Caregiver Fund” in the donation form.
