Important Communication Update
Beginning July 14, 2026, Southcoast Health will introduce a new communication platform designed to make it easier to stay informed and connected to your care. This enhanced system will support secure two-way text messaging and improved phone communications between patients and Southcoast Health.
With this update, text messages from Southcoast Health will transition to our new text message short code 639104 and our new phone number 508-970-9470. We encourage patients to save this number to their contacts so they can easily recognize communications from Southcoast Health.

Referrals to a Southcoast Physician
Your primary care physician is responsible for evaluating your condition or illness and determining the best course of treatment. Should you require a referral, Southcoast Care Connect can assist you in choosing the right specialist, getting an appointment and making sure your visit is authorized by your insurance.
Prior to your appointment, please be sure that any specialist you see participates in your insurance plan to avoid non-coverage of service.
What to do to prepare for your visit

Your MyChart patient portal is a seamless way to access all your appointment information and more.
Arrive 15 Minutes Early
Please arrive 15 minutes early to your appointment when possible. If you are running late, please call the office.
If You Need to Cancel
We kindly ask that you cancel your appointment via MyChart or by calling the office no later than 48 hours before your scheduled time.
Check Insurance
Ensure your insurance carrier has your primary care physician listed.
Bring Necessary Documents
- Photo ID | Insurance card(s)
- Copayment or payment for services rendered
- Medication(s) you are currently taking in their bottle(s)
- If applicable – Medical History Form and past medical records
