Locate a nearby physician or specialist that meets your health needs.

MyChart

Easily refill your medication, access your health summary and more through your online MyChart profile.

If you’ve forgotten your ID or password, reset it by completing one of the forms:

Online Registration

Register before you even walk in the door for your upcoming doctor’s appointment with our online check-in.

CareConnect

Talk to one of our local call center operators at 844-744-5544 to find a doctor in your area.

Medical Records

Contact us to request a copy of your medical records. You can also log on to MyChart to view your health summary.

Patient Financial Services

Southcoast Health offers patient financial counselors to help determine your eligibility for health insurance programs.
