How to Get to Your Medical Care

  • Walk or drive to your primary care provider’s office using a private car, bus, taxi, or chair car service that may be available to you
  • If you need transportation to a follow-up visit due to a recent hospital stay, please contact 774-510-0014 at least 48 hours before the appointment to request transportation assistance
