Partnering with Your Primary Care Physician for Elevated Care

Once you are admitted into a Southcoast hospital, we will send your primary care physician a notification, and stay in contact with them while we collect your medical records and perform tests. Following your discharge, all of your inpatient information will be available to your primary care physician.
