Promoting Healthy Communities with Mobile Health Services
The Community Wellness Program (CWP) is the next phase in our mobile health services to better serve the needs of our communities. With two electric vehicles, we will improve access to healthcare and health equity across the South Coast Region.



Services & Hours of Operation:
The CWP will continue to provide outreach and education to the underserved and vulnerable populations in our community. Services will also include preventative screenings, vaccinations, and screenings for health related social needs such as access to food and affordable housing, with referrals to appropriate resources.
The standard hours of operation are Monday – Friday from 8:30 am to 5:00 pm, with flexibility when requested to provide services during evening hours and on the weekend.

Every effort will be made to return calls within 24 hours of a message being received during standard business hours. When after hours or on weekends, return calls will be made the following business day.
Meet the Team:
The CWP team is made up of a diverse group of individuals who are passionate about serving our communities.
Registered Nurse:
Stephanie Perry has been an Addiction Nurse Specialist for over 8 years, with an experienced skill set and an extreme passion for those struggling with mental/behavioral health needs, substance use disorder, and homelessness.
Licensed Practical Nurses:
Diana Camara has 15 years of experience working with the developmentally disabled and underserved populations, assisting with education and life skills development. She is a graduate of the Diman Regional LPN Program and is fluent in Portuguese.
Jianna Heydt graduated as a LPN in 2020, Jianna is continuing her education and career path to become a RN.
Community Outreach Specialists:
Barbara Durand has been in the human service field for over 25 years, and has a background in mental health services, by providing support to patients during their recovery and connecting individuals with housing assistance.
Carla Marcelino has worked in the healthcare field for over 15 years and has been a certified Community Health Worker in the South Coast region for over 5 years, with expertise connecting underserved individuals to health care services. Carla is fluent in Portuguese and Spanish.
Host-Site & Outreach Opportunities:
The CWP will provide selected services, education and outreach opportunities in partnership with a community-based organization, also known as a host-site.

Southcoast Health promotes community health & wellness through the Community Wellness Program, serving people consistent with the Southcoast Health primary service areas. This includes Greater Fall River, Greater New Bedford and Greater Wareham, MA communities.
Southcoast Health Wellness Van:
After 25 years of service and breaking down traditional barriers to care through health screenings, education, vaccinations Southcoast Health has retired the Wellness Van vehicle and transitioned the services to the Community Wellness Program. Southcoast is grateful for the previous team’s dedication to serving the community.
