Pre-registering online is an easy, convenient way to expedite your hospital visit.

There are two basic steps to follow:

Step 1: Collect the information you will need.

Step 2: Complete the Secure Pre-Registration form.

Your online information will be processed by the end of the next business day. You will receive a confirmation e-mail once it is processed.

Protecting Your Privacy

Step 1 | Collect Your Information

Start by collecting all the information you will need to complete the Pre-Registration form:

• All insurance cards

• Employer information

• Name, address and phone number of next of kin

• Name, address and phone of a person to notify (different from next of kin)

• Name of Doctor that ordered your exam

• Reason for you visit

• Date of your appointment

Step 2 | Complete the Pre-Registration Form

Once you have collected all the information above, it's time to fill out the form online. This will take about 10 to 15 minutes.
