Southcoast Diabetes Management Program relocates to new Wareham location
WAREHAM, Mass. — Southcoast™ Diabetes Management Program, part of Southcoast Health System, will relocate its Wareham office from Tobey Hospital to Southcoast Health System at Rosebrook, 100 Rosebrook Way in Wareham. Diabetes management services will be available to patients at the new Rosebrook location beginning March 19. The transition to Southcoast Health System at Rosebrook will be seamless for patients, requiring no action on their part.
Part of Southcoast Health System, Southcoast Diabetes Management Program is an American Diabetes Association-certified program that offers a comprehensive series of educational sessions designed to give adults and their families the knowledge to live healthy lives with diabetes. Education is offered in small group sessions or on an individual basis. The program addresses all aspects of diabetes self-management, including continuous glucose monitoring program, insulin pump use and training, self management skills, gestational diabetes management (development of diabetes during pregnancy), medication management, medical nutrition therapy, basic meal planning, personal sensor education and insulin injection instruction.
The program is staffed by a team of certified diabetes educators that includes both nurses and dietitians who are committed to helping patients live a healthier life.
The Diabetes Management Program accepts most major health insurances and is now accepting appointments by physician referral. The Southcoast Diabetes Management Program also has locations at the Durfee Union Building, 283 Pleasant Street in Fall River; the Mashpee Building, 49 State Road in Dartmouth and at Linden Tree Family Health Center, 2444 East Main Road in Portsmouth, R.I.
