Code A
(Code Assist)
All of our caregivers at Southcoast are dedicated to making every patient's hospital stay safe. We
want you to be a partner in the delivery of care.
There are times when you are concerned that there is a serious change in a patient's condition that may be a medical emergency.
We have created "Code A" to address our patients' needs and concerns in these cases.
Use "Code A" to call for immediate help when you feel there is a serious change in a patient's condition.
When to Call
You notice a serious medical change in the patient that the health care team is not aware of, such as:
- Difficulty breathing.
- Seizure.
- Confusion or change in mental condition.
- Something is just not right.
How to Call a Code A
To access Code A, call 4357 (HELP) from any hospital telephone.
You will be asked for:
- Room number.
- Patient name.
- Patient concern.
- Your name.
We will then immediately activate "Code A" and dispatch a Rapid Response Team.
Rapid Response Teams
Rapid Response Teams are multi-disciplinary teams of critical care specialists that gather at a patient's bedside to assess that patient's condition and advise on the best way to proceed with the patient's care.
