As a patient of Southcoast Health, we want your health care experience to go as smoothly as possible. Easily find the information you need to request medical records.

Please allow at least five business days after receipt of your fully completed form for most routine medical record requests to be fulfilled. Urgent requests should be made by phone. Fees may apply for copies of some medical records.

Patients, did you know you can access your medical records online? If you are a current patient, you can view your nonsensitive test results through MyChart, Southcoast Health’s patient portal.

Obtaining Hospital, Urgent Care, Oncology, Radiology/Imaging, and Physical Therapy Medical Records

Medical records are maintained by the Health Information Management Departments for all three hospitals. To obtain a copy of your medical record:

1. Download the Authorization for Release of Information Form in  English, Portuguese or Spanish.
2. Print and complete the authorization form to release medical records.
3. Mail, email or fax the completed form to:

Obtaining Southcoast Physicians Group Medical Records

2. List your doctor’s name and office address in section 3 of the form.
3. Print and complete the authorization form to release medical records.
