E-mail a Patient at Charlton
You can send an e-mail to a patient at Charlton Memorial Hospital simply by filling out this form and clicking send.
Your message will be printed and delivered by our volunteer coordinators Monday through Friday and delivered up until about 4 p.m. Messages received after hours and on weekends and holidays will be delivered the following business day.
Although we will do our best to deliver messages the same day, there may be circumstances such as periods of high patient volume when delivery may be delayed.
Things to remember:
- Your message will not be confidential, due to the delivery method. We recommend you not send content that is sensitive in nature.
- We reserve the right to review incoming e-mails for inappropriate content. We will not deliver messages that use profanity, alarming or offensive language.
- Remember that a lot of names sound the same. Please be careful and accurate and provide as much detail as possible.
- While we will make every effort to deliver your message to the person you intend, we are not responsible for a message being misdirected to another patient with a similar sounding name.
